I authorize Apex Reel Inc. to feature the above-named player on Apex Reel's official social media platforms — including but not limited to Instagram, Facebook, and YouTube — for the purpose of showcasing player profiles, highlight reels, and recruiting announcements. Any content posted will be professional in nature, used solely to promote the player's recruiting profile, and will not include personal contact information. Content will not be used in paid advertising without separate written consent.

I authorize Apex Reel Inc. and its commercial partner Soccer World Central to display highlight reels, player profile assets, and Apex Reel promotional content featuring the above-named player on in-store television screens and digital displays at Soccer World Central retail locations. This use is strictly for non-paid, in-store promotional purposes to raise visibility within the soccer community. No content will be broadcast, streamed publicly, or used in paid advertising without separate written consent.

I grant Apex Reel Inc. the non-exclusive right to use photographs, video footage, and likeness of the above-named player for the purposes outlined above and/or solely for professional recruiting and scouting purposes. This includes use within the player profile document, highlight reels, and digital recruiting assets. Apex Reel Inc. will not sell, license, or distribute any media to third parties for commercial gain without prior written consent. All media is used exclusively in the context of player recruitment and professional development, and I agree not to remove or obscure any credits, watermarks, or proprietary notices embedded in the work by Apex Reel Inc. The completed reel and all finished deliverables remain the creative property of Apex Reel Inc. In no circumstances may the client, player, parent/guardian, or any third party acting on their behalf re-edit, re-cut, alter, manipulate, or otherwise modify the completed work, in whole or in part, nor provide, share, license, or make it available to any other business or third party for reworking, re-editing, or commercial use, without the prior written consent of Apex Reel Inc.

I consent to Apex Reel Inc. collecting, storing, and using personal information provided in this form — including but not limited to name, contact details, date of birth, academic records, and athletic statistics — for the purpose of producing recruiting assets and to provide to coaches, scouts, or institutions or similar individuals or organizations as part of the recruiting process agreed upon. Personal information collected will not be shared with any other third parties or used for any further purposes without player or parent/guardian prior consent.

I acknowledge that Apex Reel Inc. uses artificial intelligence (AI) and automated software tools as part of its production process. These tools may assist with tasks such as video clip selection and editing, player tracking and telestration, the design and layout of profile and stats assets, and the drafting of written copy or captions. AI is used to improve the speed, consistency, and presentation quality of deliverables. AI is never used to fabricate, alter, exaggerate, or misrepresent the player's actual on-field performance — all footage reflects genuine game or training action, and every deliverable is reviewed by Apex Reel before it is finalized. I further acknowledge and agree that footage and information provided to Apex Reel Inc. may be shared with and/or processed by trusted third-party AI or software services solely for the purpose of producing your assets.

Where the above-named player is under the age of 18, a parent or legal guardian must provide consent on behalf of the minor. By agreeing below, the parent or legal guardian of the minor identified above confirms they are the parent or legal guardian of the minor listed, that they have read and understood the consents and waivers described in Sections 10 and 11 above, and have the authority to, and hereby agree to, grant the waivers and consents described above on behalf of the minor, and that all information provided above is accurate and truthful.
